Health and social care service is a critical one where the health care professionals and other service providers need to be efficient and cooperate with each other in order to provide high quality service after diagnosing the patients with care (Aveyard, 2014). The aim of the report is to understand personal values and principles as well as individuals skills which are required for delivering quality services as per the needs and preferences of the individuals. Through this report, it is possible to develop personal skill and knowledge in order to work with others and improve patient’s value by delivering high quality health care service as per their health needs (Brett et al., 2014).

As a health and social care provider, it is necessary to develop personal value for delivering high quality service to the patients. In this regard, respecting the individuals is the personal value through which it is possible to acknowledge the actual needs and preferences of the patients. In this regard, it is necessary to communicate with the patients so that the health care provider can identify the health conditions, previous health records and their preferences. On the other hand, principles of health and social care are such as maintaining equality, confidentiality, diversity management, protection of the patients and maintaining the legislative structure in the health and social care institution. For example, tension is one of the factors that may adversely affect the personal value of the health care provider. In order to manage the issue of tension and follow the principles of health and social care, I aim at handling the job calmly and solve the problems. In order to maximise personal values, it is also necessary to communicate so that the heath care professional can acknowledge the actual need of the individuals and respect them so that it is possible to build strong relationship (Glasby, 2017). In this regard, I also try to maintain positive attitude and communicate with other staff in order to manage tension and in this regard cooperation and team work can help the service providers to resolve the tension in the workplace and it provides a scope to maximise personal values as well as principles of health and social care.

Personal values have the best influence on the personal beliefs and attitude. In this context, I try to be respectful for others and I believe I should speak for the right as well as wrong things happened beside me. Positive attitude and honest behaviour are important and I belief that through these factors, I can develop strong corporate relationship with other members. I also belief that the questioned in the health and social care should be asked for improving understanding and needing proper clarification and apart from that I demand empowerment of the people so that all the members can participate in the organisational decision making behaviour and it further provides me a scope for enhancing the overall performance in the health and social care. I try to be friendly, cooperative and caring with the people as well as promote equality and reduce discrimination in the workplace. Furthermore, I try to improve the human rights and try to be empathetic, sympathetic and treating the people with respect. Moreover, maintaining privacy and confidentiality as well as managing transparency are also helpful for me to improve personal values and culture so that it is possible to support the health and social care proficiently.

Assess how personal culture and experience influence own role in supporting users of services and others in health and social care settings

Personal culture and experience in this regard is also effective to deliver high quality health and social care service to the patients as per the health needs and preferences of the individuals. In this regard, personal culture of the health and social care professional includes influencing people, building strong relationship and managing different ethnicity group in the health and social care institution. Influencing people is helpful through which I aim at motivating the patients as well as encourage other team members to work properly in order to meet the principles of the health and social care. In addition to these, building strong relationship and maintaining positive attitude in the workplace are my personal culture and experience through which I can manage the patients successfully as well as develop effective team in the workplace for delivering high quality services (Dickinson and O'Flynn, 2016). In addition to these, I also try to maintain confidentiality as well as manage diversity and follow legislations in the health and social care in order to support the users successfully.

The individual culture, way of treating people, the type of training and communication practice has direct influence on the behaviour of the health and social care provider. As per the family culture, I show love and build corporate relationship with other team members and apart from that, I try to respect all the people irrespective of their language, culture and religion. It is my duty in this context to support equality and mitigate the issue of cultural diversity in the workplace. I focus on fair treatment, transparency and accountability for improving effective organisational culture where the people can work with freedom and harmony. It is my code of practice where I try to communicate with all of the colleagues as well as support them for more cooperation and maximisation of their values. My way of communication through interaction with the family members by ensuring love and respect are also helpful for my career where I am successful in acknowledging the personal needs and preferences of the patients for improving their quality of lives.

Discuss how new developments and changes to personal values can impact on work in health and social care

In order to develop new personal values, the health and social care professional needs to develop communication skill so that it is possible to deliver quality services after acknowledging the actual health condition and requirements of the patients. In this regard, it is necessary for me to improve technical skills and utilise the Information and Communication Technology (ICT) in the health and social care institution in order to improve external and internal communication. Through the ICT technology, I try to interact with the patients for collecting authentic information of the individuals so that I can diagnose the disease. On the other hand, it is useful for enhancing internal communication among the nurses, practitioners, doctors, other staff members and health care providers which provides me a scope to improve team work and treat the patient with quality care. In addition to these, it is also useful for me to maintain legislations such as Health and Social Care Act 2012 and Equality Act 2010 to enhance value for the patients. It also provides me a scope to improve patient’s care as well as helps me to treat the patients equally (Glasby and Dickinson, 2014). Additionally, good practice, positive attitude, managing tension at the workplace as well as communication and interaction with the patients are helpful for me to meet the principles of the health and social care and improve involvement with the patients which is necessary to deliver quality services to the individuals.

Be able to produce, monitor, revise and evaluate plans for personal progress in developing the skills and abilities required of a health and social care practitioner

Assess current skills ability and learning style

Through personal analysis, it is possible to understand the abilities and learning style of the health and social care provider. Working experience, good networking skill as well as the emotional intelligence are the main strengths of the health and social care professional and through these strengths, it is possible to provide quality care to the patients after suitable diagnosis. Positive attitude, respecting others and decision making behaviour are also my strengths which helps my professional and personal development. On the other hand the main weaknesses are lack of communication and poor interpersonal skill and these may adversely affect the quality of care of the health and social care institution. In this regard, the quality of the services is dependent on the nursing abilities of the staff members (Krachler and Greer, 2015). The main threats are managing obstacles in attending training as well as low ability to develop skill. Enhancing personal development, improving quality of care, attending more training and development program are the major opportunities which further provides a scope to the health care provider to develop personal skill and manage the organisational task successfully. As per the Honey and Mumford’s learning style, the service provider is like activist where the person is interested in doing and experiencing. After observing and reflecting the situation, the service provider tries to develop own skill and gather experience successfully.

Produce a holistic development plan with short medium- and long-term goals

In order to develop personal skill, the action planning is important where the service provider needs to improve interactive skill, verbal and non-verbal communication so that it is possible to interact with others in the care institution as well as with the patients. Managing conflicts and improve team building activities are also important for developing personal skill (Reeves, and Harris, 2016). In addition to these, the provider needs to improve decision making and problem solving skill for taking proper decision for the patients in order to maximise their values. Considering the SMART objective, the objectives are specific as the service provider needs to improve interpersonal skill to create values for the patients. The objectives are measurable the outcome of the objectives can be assessed which is to maximise patient’s care. The goals are also attainable as the service provider can achieve the objectives by utilising his or her strength and working experience. It is realistic as it is necessary to improve personal skill to deliver quality care. The goals can also be achieved within the above mentioned time.

Monitor progress against the plan according to the requirements of a health and social care practitioner, revising the plan as required

The health and social care service provider tries to assess own progress through discussing with other experts in the institution. Self-assessment as well as evaluating the performance of the service provider is the main strategies for monitoring the progress. The planning proves further encourage the service provider to improve interpersonal skill and deliver high quality service to the patients after acknowledging their needs through interaction.

Evaluate the effectiveness of the development plan to own development as a health and social care practitioner

The development planning is effective as it provide a scope to improve communication of the service provider and this has direct effect on the performance. Through improving interactive skill, the service provider can interact with the patient and improve trust and loyalty so that the individual can share personal information. This is effective to acknowledge actual health needs of the patient and apart from that the service providers can develop effective team through more communication and cooperation for working better and improving values for the patient. Through the plan I can improve my efficiency and performance in long run. It is also helpful for me to achieve the pre-specified goals and objectives. It highlights the gap of my knowledge and skill and it further provides me an opportunity to enhance my professional knowledge for performing better. Induction training as well as training and development during the work are also beneficial for me and in this regard the plan can enable me to improve my knowledge and understanding in the health and social care practice. The plan is basically a structural way of enhancing and utilising my strengths and mitigating my weaknesses so that I can utilise further opportunities in near future.

Understand the application of principles of professional engagement with users of health and social care services

Explain the nature of different professional relationships in health and social care contexts

Professional relationship in the health and social care is important for working cooperatively and maximise the values of the patients. In this regard, I maintain inter professional relationship with all the staff members, doctors, experts, heath care professionals and other nurses. I focus on improving cooperative work culture in the institution and respecting each other which further helps me to maximise the quality of patient’s care. Positive attitude and active listening are also effective for me to manage professional work place and improve cooperation (Hewitt, Sims and Harris, 2014). I am also concerned about developing effective team through cooperation and communication so that it is possible for me to deliver quality service to the patients successfully.

Evaluate personal effectiveness in promoting and supporting the rights of the individual

Personal effectiveness is useful to promote and support the rights of the individuals admitted in the health and social care institution. In this regard, I am concerned about Care Standard Act and Equality Act 2010 so that all the patients are treated fairly and it further ensures that nobody in the care institution is discriminated or abused. In addition to these, I try to be mentor and facilitator for monitoring the progress of the patients and facilitating the values for the patients by treating them effectively and providing quality care as per their health needs. In addition to these, I am also willing to be counsellor and adviser so that it is possible to advise the patients and discuss the current situation of the patients as well as motivate them to cooperate with the care provider so that the values of the individuals can be maximised (Valentine, Nembhard and Edmondson, 2015). Moreover, I also try to manage internal conflicts in the health care institution for motivating the other staff members and developing an effective team. Additionally, I try to create values for the patient which is another skill through which I can promote and support the right of the individuals so that all the patients can get quality care and effective treatment as per their health needs and preferences. I promote human rights as well as ensure transparency, accountability, empowerment of the team members, and concern about non-discrimination and improve participation and involvement.

Discuss ways to resolve issues encountered in professional relationships

The major challenges in the health and social care institution are lack of group cohesiveness, miscommunication, poor understanding regarding the job role of the serve providers, lack of team work and poor working environment in the hospitals. I try to resolve these issues and motivate the staff members so that I can work cooperatively. In this regard, I focus to be cooperative and interactive to develop effective team within the health and social care institution as well as work as a group to maximise the values for the patients. In addition to these, implementing Information and Communication Technology (ICT) in the hospitals is also necessary to enhance communication and cooperation through which I aim at improving collaboration and maximising the contribution of all the team members for providing the best treatment and quality care to the patients. Discussing about the actual job responsibility and providing proper training program are also effective as these strategies help to reduce the issues of lack of understanding of the job role of the care providers (Addy et al., 2015). Additionally, I try to be supportive and create positive working atmosphere as well as they need to be concerned regarding the safety of the workers by implementing CCTV camera, fire extinguisher and make the place hygienic and sanitary.

Evaluate the effectiveness of personal contributions when working with others in health and social care practice

In the health and social care institution, it is necessary to work with others in order to maximise the values of the patients. In this regard, personal contribution plays a crucial role in developing an effective team. My technical skill and knowledge in delivering quality care are helpful to share knowledge and understanding with others. In addition to these, my positive attitude and interactive skill further helps me to contribute positively in developing effective team. Cooperation and understanding among the team members are also contributing factors in working with others (Henderson, 2017). Managing own responsibility as well as helping others to do their responsibilities successfully are also contributing in developing an effective team where the team members can cooperate with each other to meet the team objectives. I am supportive and treat the team members fairly with positive attitude and listening their ideas. I focus on maintaining body language, eye contact and sharing information fairly as well as I aim at making decision collaboratively by empowering all the team members in the decision making procedure. Mutual support, communication and cooperation are the main strategic planning through which I try contribute with full potential when working with others in the health and social care institutions.

Explain how the limits of own work role impacts on work with others

Limits in own work role in the health and social care organisation may has crucial impacts on working with others. As the health and social care provider, I did not attend proper training and development related to the disease of the patients and in this regard lack of knowledge and understanding of the care worker may has adverse effects on maximising the role of the service provider. It is necessary for the provider to cooperate with other team members and improve knowledge and technical skill so that it is possible to work with others and maximise the values for the patients by delivering quality care. I am also suffering from lack of technical knowledge which further deteriorates my performance in the health and social care. My another limit is lack of initiatives in meeting the expectation of the patients where I am not able to understand the actual needs of the patients and current health condition due to lack of appropriate training program. In addition to these, lack of experience and different priorities of the patients are also other factors which deteriorate my efficiency as the service worker and it further affects the initiatives to work with others (Dunér and Wolmesjö, 2015).

Analyse own role in minimising barriers to effective teamwork in health and social care practice

In order to minimise the barriers to effective teamwork in the health and social care practice, it is necessary to communicate and maintain positive attitude in the workplace. Apart from that, managing transparency and accountability as well as following the Care Standard Act and Equality Act 2010 are also my tactics to develop an effective team work. The health and social care provider also needs to contribute positively and interact with others in order to share authentic information. Moreover, helping other team members and clarify the roles and responsibilities of others are also necessary where I try to enhance collaborative workplace (Reeves et al., 2011). I mainly focus on empowering others in the decision making process as well as improving interest of all the team members so that it is possible to develop cooperative work culture in the workplace. Providing effective training and enhancing communication through ICT are also effective to enhance team work in the health and social care organisation where the health care worker can improve own abilities and technical skill to treat the patent with quality care. The above mentioned tactics for minimising the barriers of effective teamwork are beneficial for my personal and professional development where I improve myself in cooperating with other team members in the organisations. the legal rules as well as technological advancement further helps to improve my understanding regarding improving team work so that all the team members will be contributing collaboratively for meeting the goal of providing best social care and health treatment to the patients.

Discuss how to improve personal contributions to the collective effectiveness of a team

In order to improve personal contribution in collective effectiveness of the team, the health and social care worker needs to manage accountability and transparency for developing trust and loyalty among the members. Additionally, discussing clear roles and responsibilities of other team members and encouraging their creativity are also effective contributing factors in improving team work in the workplace (Körner et al., 2016). The service worker also needs to manage conflicts and motivate them by providing monetary and non-monetary rewards. Managing conflict as well as positive organisational culture are therefore contributes well to manage team work and maximise patient’s care by delivering effective treatment and quality care as per the current health needs and preferences of the individuals. Sharing information, respecting each other in the workplace and friendly atmosphere as well as collective work and managing safety and security in the workplace further contribute in the effectiveness of team work.

Conclusion

The role of health and social care worker is important in improving values of the patients and developing team work in the workplace in order to enhance organisational performance. The health care worker tries to maintain health and social care act as well as Equality Act 2010 and Discrimination Act in order to make the service fair by managing transparency and accountability. Team work, positive attitude, respecting theirs, communication and cooperation in the workplace further enhance the performance of the workers in maximising the quality of patient’s care.
